Our social work teams provide a range of short and long term support, depending on the situation, urgency, and complexity of need.

COMMUNITY OUTREACH SERVICES
  • Provides information about services available for older adults

  • Completes forms and applications for government benefits, transportation, and housing needs

  • Accesses services and supports for individual situations and preferences

  • Informs about activities in the community and provides opportunities to meet new people

  • Provides support during difficult life transitions, and helps older adults with complex issues such as elder abuse, mental health challenges, financial difficulties, homelessness etc.

EMERGENCY SOCIAL WORK RESPONSE

An older adult who is at-risk or in crisis may have problems accessing food, clothing, or housing, or may be experiencing more complex issues such as mental health decline; caregiver stress; homelessness; or elder abuse.

 

A SeniorConnect worker will address immediate concerns, stabilize the crisis, and secure appropriate community supports.
